Student-Athlete:
Greetings from Huntington University! I trust
that your summer plans are taking shape and that it looks to be
a productive, yet relaxing few months for you. We as a coaching
staff are anxiously anticipating the fall and are excited to
have the opportunity to work with you.
In order to facilitate your participation in
athletics here at HU, we have put together for you and your
parents some information concerning requirements for insurance,
physicals and eligibility. The information concerning insurance
and physicals will require your immediate attention. All of the
forms that you will need are accessible through our Huntington
University Athletic Training website -
http://www.huntington.edu/athletics/training.
Beth Herrell is our NATA-certified, Head Athletic
Trainer. She is responsible for our training room and its
everyday operations. Beth is your contact person on matters
related to athletic medical care. Beth also has enlisted the
help of an excellent support group of medical doctors, physical
therapists, and orthopedic specialists, each of whom provide
excellent care for our student-athletes.
On our Athletic Training website, you will find a
link to the Huntington University Medical Protocol which begins
with a letter from Beth briefly outlining the necessary physical
and insurance forms that you must complete and submit to the
Training Room in order to be able to participate in our
intercollegiate athletic program. You may access the required
forms under the Athletic Participation Forms link. Please print
and complete each of the forms.
Once you have completed all of the forms, make a
copy of them for your files and then mail the originals to Beth
Herrell, Huntington University Athletic Trainer, 2303 College
Ave., Huntington, IN 46750. Fall sport student-athletes must
make sure that their forms are mailed at least 10 days prior
to the first day of training camp to insure that they are on
file in the training room before the team’s first practice. If
all of your forms are not on file, you will not be able to
participate in practice or competition until they have all been
submitted. Winter and spring sport student-athletes may mail or
hand-deliver their completed forms to the training room when
they arrive on campus in the fall.
