Dear Student-Athletes:
Greetings from Huntington
University! I trust that your summer plans are taking shape and
that it looks to be a productive, yet relaxing few months for
you. We as a coaching staff are anxiously anticipating the fall
and are excited to have the opportunity to work with you.
In
order to facilitate your participation in athletics here at HU,
we have put together for you and your parents some information
concerning requirements for insurance, physicals and
eligibility. The information concerning insurance and physicals
will require your immediate attention, while the brochures are
just to alert you to NAIA and HU requirements.
We
are also committed to helping you with matters related to
athletic medical care. We have enlisted the help of an excellent
support group of medical doctors, physical therapists, and
orthopedic specialists.
On
our athletic training website,
you will find two letters (one addressed to you and one to your
parents), a copy of the Huntington University medical protocol
and insurance policy, an insurance form, an NAIA eligibility
brochure, and a Huntington University guide for
student-athletes. All of the forms that you will need are
accessible through our website including the Health Form
which requires a Physical Examination from your physician.
As student-athlete, you should
send the completed insurance, ONE and physical forms to the
Health Services office. The forms must be on file before you
will be able to attend classes in the fall
and prior to any participation in
practice or competition. For student-athletes returning
for pre-season practices before the start of school, the
material MUST be returned PRIOR to your coming to campus. You
will need to bring an additional copy of your physical exam and
your insurance form with you to your first team meeting.
